Dad needs in-home care now. The practical question is not whether one professional should handle everything. It is which work the family can do, which work a VA-accredited Veterans Service Organization representative will do free, and which Medicaid questions require separate help.
Start care when it is needed and affordable on private pay. A pending VA pension claim is not a payment guarantee, and VA does not promise to decide a claim before the next care invoice is due.[1] Build a private-pay plan that works without assuming an approval date or award amount.
The three gates
First, test military service. Check Dad's discharge character, active-duty dates, entry date, total active service and whether at least one day falls within a VA-recognized wartime period. Do not rely on a campaign medal, VFW membership or the family's description of him as a wartime veteran. Compare the discharge record directly with VA's service rules and wartime table.[2]
Second, test basic pension eligibility. Dad must meet VA's age or disability requirement as well as its income and net-worth rules. Being old enough, needing care or having served during wartime does not satisfy the other gates by itself.[2]
Third, test the higher Aid and Attendance level. VA says a pension claimant may qualify when another person is needed for daily activities such as bathing, feeding or dressing, when illness requires substantial time in bed, when the claimant is in a nursing home because of mental or physical incapacity, or when specified vision limits apply. Aid and Attendance is an increased pension level, not a stand-alone home-care program.[3]

The forms
For Dad's original Veterans Pension claim, begin with VA Form 21P-527EZ. Use VA Form 21-2680 for the Aid and Attendance examination, and have the medical examiner complete the examination information. If Dad is in a nursing home, the facility may also need to complete VA Form 21-0779.[4]
VA Form 21P-8416 reports qualifying medical or dental expenses that were paid and were not, and are not expected to be, reimbursed. VA Form 21P-0969 is used when the pension application instructs the claimant to provide the additional income and asset statement, or when income or net worth must be verified or updated.[5]
If the paper packet is not ready, review VA Form 21-0966, Intent to File. VA says an intent to file can establish a potential effective date, but the completed pension application still must follow. Starting an identity-verified online pension application can establish the effective date without a separate intent-to-file notice.[4]
The family can complete the household inventory, request the discharge record, list income and accounts, collect paid invoices and schedule the medical examination. Do not ask a paid care company to decide what belongs on the VA forms merely because it sells care. Give the finished inventory to an accredited representative for a claim review.

How long
VA gives no universal decision time for a pension claim. It says timing depends on the claim and that claims generally are processed in the order received unless priority processing applies.[6] Treat every VA evidence letter as its own deadline document. Record the date received, the response due date stated in the letter, the person responsible and the submission receipt.
Ask the care provider for a private-pay schedule covering the period before any VA payment arrives. Also ask what happens if VA denies the claim, approves a lower pension level or assigns a later effective date than the family expected. Do not promise the provider that VA will reimburse a particular invoice.

Who files it free
A VA-accredited VSO representative can help gather evidence, file the claim and communicate with VA. VA states that services provided by an accredited VSO representative on a benefits claim are always free.[7] Confirm the individual or organization in VA's accreditation search before sharing financial or medical records.
For St. Louis options, use Local Help: St. Louis Offices. At the first call, ask whether the representative handles pension claims, whether an appointment is required, what records to bring, how representation will be established and how the office will provide proof that VA received the claim.
DIY does not have to mean filing without review. A useful division is: the family owns the facts and documents, the clinician owns the medical examination, and the accredited VSO checks and submits the VA claim. Paid Medicaid planning, if needed, remains a separate engagement.

Two VA doors
The Veterans Benefits Administration decides the pension claim. For claim status, filing routes and benefits questions, call VA benefits at 800-827-1000 or work through the accredited representative.[8]
The Veterans Health Administration handles VA health care. Enrollment, primary care, caregiver support and possible home or community services are separate from the pension decision. Call the VA health benefits line at 877-222-8387 to ask about enrollment and available care.[9] Approval at one door does not establish eligibility at the other.

Survivors
If Dad has died, do not reuse his Veterans Pension application. An eligible surviving spouse or child applies for Survivors Pension, DIC or accrued benefits with VA Form 21P-534EZ. The application can also request increased survivor benefits based on Aid and Attendance or housebound status.[10]
For Survivors Pension, check the Veteran's qualifying service, the claimant's relationship and marital status, and the survivor household's income and net worth. A surviving spouse's Aid and Attendance request still needs evidence of the survivor's own care need.[11]

Expenses
Build the expense file from records, not estimates. For each claimed expense, keep the provider name, person receiving care, service, service period, amount charged, amount paid, payment date, payer and any reimbursement. VA Form 21P-8416 is for paid, unreimbursed medical or dental expenses, not every amount appearing on a proposal or invoice.[12]
Ask the VSO which recurring care costs VA may consider and what supporting record should accompany them. Keep the care contract, invoices, bank or card proof, insurance statements and caregiver worksheets together. The care plan and the pension calculation should use the same household and expense facts.

Medicaid
VA pension and Medicaid are different programs with different financial rules. VA reviews household income, net worth and certain asset transfers when deciding pension eligibility. Moving or giving away assets to qualify can produce a VA pension penalty period.[13] Medicaid eligibility and transfer consequences are administered separately.
This is the point where hiring may make sense. If Dad may need Medicaid-funded long-term care, owns property beyond his residence, has a trust, made gifts, transferred an account, added a joint owner, or is considering a caregiver agreement, get advice from a lawyer who regularly handles Missouri or Illinois Medicaid eligibility before changing ownership or moving money. Ask for a written scope that identifies whether the lawyer is analyzing Medicaid, VA pension or both, and verify any person handling the VA claim through VA's accreditation search.
Do not pay someone merely to collect documents the family can collect or to file an initial claim a VSO will handle free. Pay, if appropriate, for the separate legal analysis the free VA claim service does not provide. Bring that analysis back to the VSO so the VA forms report the transactions and household finances consistently.
The working order is simple: screen all three VA gates, price care without assuming an award, assemble the records, obtain a free accredited VSO review, and pause before any asset transaction that could affect either VA pension or Medicaid.
