Who fills out VA Form 21-2680?

The examining physician. That is the only honest sentence. VA's own form page tells you to use 21-2680 to apply for Aid and Attendance added to compensation or pension. It does not invite the son, the neighbor, or a salesman to complete the exam. Your job is logistics: download the current PDF from VA.gov, put it in a folder with the DD214, and take it to the next appointment. Ask the office how they handle VA exam forms. Some will complete it in the visit. Some will want you to leave it and pick it up. Some will want a dedicated slot. Ask. Do not guess, and do not coach the wording.

What the form is asking, in plain language, is whether the person needs regular help with daily activities or is housebound. That lines up with how VA describes the benefit: monthly payments added to a pension for qualified Veterans and survivors who need help with daily activities such as bathing, feeding, and dressing, or who are housebound. The doctor already knows what he can and cannot do in the exam room. Your one-page note of what happens at 2am at home can be useful context for the appointment. It is not a substitute for the exam, and it is not a diagnosis you are making.

If the person is in a qualifying extended care facility, VA also publishes VA Form 21-0779 for a nursing home official to verify that. That is a different piece of paper. Do not mix them.

What other forms sit next to 21-2680 on an Aid and Attendance claim?

The doctor's page is not the whole file. For a wartime Veteran filing pension, the application is VA Form 21P-527EZ, Application for Veterans Pension. VA's form page lists the related paper in the same stack:

Should I wait for the doctor to finish 21-2680 before I tell VA I intend to file?

Usually no. VA says to use 21-0966 if you are still gathering information and want to start the filing process, and that submitting an intent to file can secure the earliest possible effective date for any retroactive payments you may be eligible to receive. On the how-to-apply page, VA adds that if they approve the claim, retroactive payments are payments for the time between when they processed your intent to file and when they approved the claim. If you sign in with an identity-verified account and apply for pension benefits online, you do not need a separate intent to file.

That is why a VSO will often file the intent, or the application, before the exam comes back. Waiting on the doctor's office to "get around to it" can move the effective date. The exam still has to be done. It does not have to be done before you tell VA you are coming.

What 21P-8416 is doing in this stack

Families skip the expense form and then wonder why the income gate failed. VA uses unreimbursed medical expenses to reduce countable income. The regulation is 38 CFR 3.278: payments must be unreimbursed to be deductible, and in-home attendant payments for help with daily activities can count when the rule's conditions are met. That math is a separate briefing. The form you actually fill is 21P-8416. Keep invoices. Keep a simple hour log. Do not invent a spreadsheet of "what the VA will pay." Document what you already paid and were not paid back.
