Aid and Attendance vs Housebound is two add-ons on the same pension, not two programs you stack. You cannot receive both at the same time. The doctor writes the need. This desk does not.

Aid and Attendance or Housebound benefits are monthly payments added to a VA pension for qualified Veterans and survivors who need help with daily activities such as bathing, feeding, and dressing, or who are housebound. Two findings. One pension. One add-on at a time. It is cash, not a voucher sent to a care company.

What is the difference between Aid and Attendance and Housebound?

Both ride on the Veterans Pension or the Survivors Pension. Both are monthly cash added to that pension if the file clears. The names describe different need findings. Aid and Attendance is the add-on VA lists for qualified Veterans and survivors who need help with daily activities such as bathing, feeding, and dressing. Housebound is the add-on VA lists for people who are housebound. This desk will not invent extra clinical tests or a homemade scorecard. VA publishes both on the same page. Take that page to a VSO. Take the exam form to the physician who treats him.

The form is the same for both findings. VA Form 21-2680 is the Examination for Housebound Status or Permanent Need for Regular Aid and Attendance, used to apply for Aid and Attendance benefits that will be added to monthly compensation or pension. The examining physician completes it. You bring the blank PDF. You do not write the medical findings. The 21-2680 briefing is the logistics page.

Need is gate two. It is not the whole file. VA's eligibility page is gate one and the start of gate three: wartime service, discharge, and the age-or-disability door for the pension itself. A man can be housebound in the ordinary sense of the word and still fail wartime service, or fail net worth. The wartime periods are on that eligibility page. The three-gates page walks the Korea and Vietnam lines. One day during a listed wartime period is the wartime piece. It is not a need finding.

Can Dad get Aid and Attendance and Housebound at the same time?

No. VA's public sentence is the one this desk will not improve on: you cannot receive Aid and Attendance and Housebound at the same time. Do not file two add-on claims as if they were stacked. There is one pension. There is, at most, one of these two add-ons on it.

That is why the exam form is one page with both names in the title. The physician is being asked to examine him for housebound status or for a permanent need for regular aid and attendance. Which finding, if any, belongs on the form is a medical question. This desk does not pick it. Bring a one-page note of what he cannot do alone at night. That note is context for the appointment, not a request that the doctor tick both boxes.

If the person who needs the add-on is a surviving spouse, the pension is still paid to the surviving spouse. They decide how to spend it. The add-on question is the same shape: Aid and Attendance or Housebound, not both. The surviving-spouse path is the survivors briefing.

A different VA door is not a workaround for "both." A Homemaker and Home Health Aide is a trained person, supervised by a registered nurse, who comes to a Veteran's home to help with personal care and daily activities. That is VA health care, not a second pension add-on. PCAFC is another door: the Veteran must have a VA disability rating of 70 percent or higher, need at least 6 months of continuous in-person personal care services, and be enrolled in VA health care. An eligible Veteran may appoint 1 Primary Family Caregiver and up to 2 Secondary Family Caregivers. That is not Aid and Attendance, and it is not Housebound.

What is the MAPR difference for a Veteran with no dependents?

This desk quotes one row. For a Veteran with no dependents, VA lists the Maximum Annual Pension Rate as $17,441 without Housebound or Aid and Attendance, $21,313 with Housebound, and $29,093 with Aid and Attendance. Maximum Annual Pension Rates increased 2.8 percent effective December 1, 2025. Those are annual figures on VA's table, not a monthly number this briefing will invent by dividing. Open the table for any other household size.

Net worth is the other money gate. From December 1, 2025, to November 30, 2026, the net worth limit to be eligible for Veterans Pension benefits is $163,699. If you are reading this after November 30, 2026, open that page again. Do not move money to manufacture eligibility. If someone is pushing an annuity as the price of filing, read the free-filing page before you sign anything.

Care you already pay for can change the income math. 38 CFR 3.278 says payments for medical expenses must be unreimbursed to be deductible. Payments for assistance with ADLs and IADLs by an in-home attendant are medical expenses as long as the attendant provides health care or custodial care, and the payments match the hours. The attendant must be a health care provider unless the disabled individual needs Aid and Attendance or is housebound, or a listed clinician states in writing that the care is required. The form is 21P-8416, Medical Expense Report. Keep invoices. The UME briefing quotes the regulation.

The Housebound or Aid and Attendance finding on 21-2680 is also one of the conditions under which attendant payments can count as medical expenses without the attendant being a licensed clinician. That is why you do not skip the exam because "he is obviously housebound." Write the hours. Get the form done.

Same form, same three gates, the doctor decides

The claim does not start with a family vote on which add-on sounds better. It starts with the pension application and the exam. For a wartime Veteran, VA tells you how to apply online, by upload, by mail, in person, or with an accredited representative. An intent to file can protect the effective date while you gather the DD214 and the doctor's page. In this metro, start with a Missouri Veterans Commission service officer or an accredited representative. They file. They do not sell products.

While the exam and the form move, someone still has to be in the house. Medicare does not pay for 24-hour-a-day care at home, or for custodial or personal care when that is the only care needed. Missouri describes a private-pay door for aide personal care, respite, or companion care. If he is enrolled in VA health care, a social worker at VA St. Louis Health Care is the right person to ask about Homemaker Aide hours. The two-doors briefing is on this site. What to do tonight

1. Write a one-page note of what he cannot do alone: bathing, dressing, feeding, transferring, nights. That note is for the doctor and the VSO. It is not a diagnosis, and it is not a request for both add-ons.
2. Download a blank Form 21-2680. Print two copies. Put one in the appointment folder. Do not fill the medical findings.
3. Open the VA pension rates table and look at the Veteran-with-no-dependents row if that is his household. If it is not, look at the row that matches and leave the math to a VSO.
4. Call a VSO in the morning about the pension application and an intent to file. Do not wait on the exam to start the date. Do not transfer assets, sign an annuity, or pay a fee to file.

When to ask the care team or the doctor

Whether he needs regular help with bathing, dressing, feeding, or whether he is housebound, is a question for the physician who treats him. This desk does not decide it. The examining physician completes Form 21-2680. Ask the office how they handle VA exam forms. Do not coach the wording, and do not ask the doctor to find both Aid and Attendance and Housebound. If he has fallen, if he is newly confused, if he cannot get from bed to chair, call the care team or the doctor. Aid and Attendance and Housebound are benefits questions. A change in the body is a medical question.
